Provider Directory
The CBHNP Capital Area and NorthCentral Region Provider Directories are available in hard copy. Please contact Provider Relations at 717-671-6500 or 1-888-700-7370 for a copy of the Directory for your county or area. The Directory is also available electronically below.
Network Providers noting inaccuracies in their information as printed in the Provider Directory should complete a "Provider Data Update Form" and submit the correct information to Provider Relations as soon as possible. This document was updated 6/28/06 to correct a typographical error on a CPT code. Please be aware that the codes that have printed on authorizations are correct.
This report will provide the names, addresses, phone numbers, county, and credentialing status of all school based outpatient sites in the Capital Area (Cumberland, Dauphin, Lancaster, Lebanon, and Perry Counties) and the NorthCentral Region (Bedford, Blair, Clinton, Franklin, Fulton, Lycoming, and Somerset Counties).
